Inpatient admission: a window of opportunity to address multiple long-term conditions

20 Oct 2026
Clinical Theatre

Hospital admissions provide valuable, yet often overlooked, opportunities to identify and address unmet healthcare needs in people living with diabetes and multiple long-term conditions.

Drawing on national evidence, real-world inpatient data and emerging clinical research, this session explores how healthcare professionals can make better use of hospital encounters to improve cardiovascular, renal and metabolic health for people living with diabetes. Topics include identifying multimorbidity, opportunities for treatment optimisation, safe inpatient initiation of cardiometabolic therapies, and the importance of effective discharge planning.

The session will also highlight the essential roles of multidisciplinary collaboration, integrated healthcare systems and communication across care settings in achieving sustainable improvements beyond hospital discharge.

Speakers
Jason Cheung, Consultant Diabetologist (Inpatient Diabetes Care) - Norfolk and Norwich University Hospitals NHS Foundation Trust
