This session presents a real-world case study of the Lewisham LTC Integrated Neighbourhood Team (INT): how it was co-designed and implemented, how a population health dashboard is used to risk-stratify and proactively identify patients who would otherwise go undiagnosed or undertreated, and what early evaluation, including patient feedback, is telling us about impact on patient outcomes. Beyond the successes, this talk will honestly address the practical challenges of neighbourhood-level delivery, including the friction points between primary and secondary care integration, and look at what the future holds for the Lewisham LTC-INT.
