CVD modern service framework: Time for a step change in CVKM management

This session examines how the MSF’s ambitions require step change improvement in detection and management of the major cardiovascular-kidney-metabolic risk factors.

The session will explore how to enable PCNs, neighbourhoods and ICBs to deliver that step change, with easy to action data that identifies all patients with CVKM conditions who are not on guideline-directed treatment, together with structured support for primary care teams to work differently in order to optimise at scale and within capacity.
