Ciara Doherty is Lead Integrated CKD Prevention Pharmacist (Cardiovascular, Diabetes) working across South East London on the Multimorbidity model of care project. With a strong focus on cardiorenal metabolic care, Ciara is committed to improving outcomes for patients at risk of chronic kidney disease, cardiovascular disease, and diabetes through early identification and optimisation of modifiable risk factors as well as providing patient and staff education.
